What is Perianal Abscess?

Perianal abscess is a condition with a painful collection of pus in and around the anal canal. It happens mostly because of infection of the anal glands situated between the anal sphincters.

The abscess can vary in size and severity and can be classified as superficial or deep depending on its location.

This can be a prior stage of developing fistula in ano.

Perianal Abscess

Symptoms to watch for

  • Severe throbbing pain near anus
  • Visible swelling and redness
  • Fever and chills
  • Painful sitting or walking
  • Pus discharge if it bursts

Common causes

  • Blocked anal glands
  • Bacterial infection
  • Crohn's disease
  • Diabetes
  • Suppressed immunity

Treatment at DLPC

Treatment involves incision and drainage of the abscess to remove pus and infected material.

As these abscesses are usually due to infected anal glands, excision of the anal glands is very important for a radical cure.

In most cases there is an internal opening of the abscess in the anal canal, which, if left untreated, can lead to recurrence and subsequent fistula formation (requiring a second surgery). We aim to locate and excise the internal opening in the first surgery itself to avoid recurrence and fistula formation.

Why treat early — Implications

  • Long-standing abscess may cause mild-to-moderate fever, extreme pain and discomfort, and spread in the perianal region.
  • If untreated, it can spread to surrounding structures or deep into the perineum.
  • May burst into the rectum or onto perianal skin to form a fistula, which is difficult to treat and may need complex surgery.
  • Recurrence and subsequent fistula formation may occur, requiring a second surgery.
